Requesting medical data

Copy, modification and deletion of your medical file
You have the right to inspect and receive a copy of your medical file within the period of the statutory retention obligation.

If you have been in contact with the Amsterdam GP Emergency Stations (this can also be a telephone contact), your own GP will always receive a notification. This states, for example, what advice has been given. Or which condition or injury has been diagnosed by the GP at the GP medical station. It also states which medications, if any, have been prescribed.

For a copy of your contact with the Amsterdam GP Emergency Stations, you can contact your own GP or submit a written request to the complaints officer of the Amsterdam GP Emergency Stations. To ensure that the request for access is made by the patient, it is required to include a copy of the patient’s proof of identity with the request. We hereby ask you to black out the passport photo and the Citizen Service Number (BSN) in this copy. This is to protect privacy. You can also use the central government’s KopieID app for this.

Furthermore, personal data is recorded when recording the telephone conversations. As a result, the recordings are subject to the duty of confidentiality and to the other privacy rules. On this basis, you have the right to listen to the conversations. You can also request a copy of the recording. You will then receive a transcript of the conversation. Audio recordings are not provided due to the privacy interest of the employee. The right to listen to the recording and get a transcript only applies to conversations between the patient and the institution.

Furthermore, you have the right to correction under the law if the factual information included is incorrect. Finally, you can submit a request for the destruction of your file. The Amsterdam GP Emergency Stations may refuse the latter with reasons. A request for annulment must always be submitted in writing. A copy of your proof of identity is also required in this case. The Amsterdam GP Emergency Stations will respond to your request as soon as possible, but within four weeks.

Request for compensation
Do you think that mistakes have been made in your contact/treatment at the Amsterdam GP Emergency Stations? And do you suffer damage as a result? Then you can hold the Amsterdam GP Emergency Stations liable (responsible). You can do this by sending a letter or e-mail to the complaints officer of the Amsterdam GP Emergency Stations. In this letter, indicate what happened, what you think went wrong and what the damage is exactly.
